Humana Inc. through Center Well Home Health seeks a Home Health Physical Therapist to help patients regain strength and independence in their homes. You will report to the Clinical Manager and deliver personalized care focused on functional improvement to enhance quality of life.
You will plan and administer PT treatments onsite, document progress, supervise aides, and coordinate with physicians and the care team. A valid PT license and CPR are required; one year of PT experience is preferred.
