Position: Registered Nurse (RN Clinical Manager - Homecare

Registered Nurse (RN Clinical Manager – Homecare)

Schedule:

Monday – Friday – 8:30a.m.–5:00p.m. One weekend only every 6–8 weeks for manager escalation calls. Hybrid schedule: 2 days in office / 3 days at home for one week, followed by 3 days in office / 2 days at home the next week. Agency:

Key Responsibilities
  • Ensure quality and safe delivery of home health services.
  • Collaborate with intake, providing daily direction and managing referral volume based on staffing availability.
  • Provide oversight for liaison workflow to ensure timeliness of completion and identify education needs for field staff and liaisons.
  • Ensure Start of Care compliance.
  • Focus on outcomes and monitor as assigned.
  • Implement quality improvement programs in the field.
  • Participate in all liaison/administrative on-call rotation and patient staffing as needed to manage after-hours team and patient-related issues.
  • Manage the on‑call calendar for internal and field staff.
  • Manage PTO, call‑offs and daily/weekly productivity reviews to ensure proper staffing coverage.
  • Complete field visits as needed.
  • Manage cost of goods by monitoring supply costs, overtime, staffing, bonuses, and visits per episode.
  • Collaborate with the billing department to address authorization requests and ensure patient contact regarding authorization exceptions.
  • Perform new and ongoing liaison training as defined by the Administrator.
  • Manage the staffing budget, including overtime and bonus oversight, report budget exceptions, and process payroll entry of field staff bonuses.
  • Provide the Administrator/DON objective data related to internal and field employee performance evaluations.
  • Maintain positive working relationships with referral sources including physicians, hospitals, skilled facilities, assisted living facilities, and families.
  • Assist with monthly Case Manager, Nurse, and Therapy meetings.
  • Review clinical documentation in accordance with agency protocol and Medicare/Federal guidelines.
  • Process complaint workflow, reporting staffing trends, neglect, abuse or theft to the Administrator for resolution and follow‑up.
  • Complete field clinician check‑in reviews in a timely manner.
  • Maintain an active RN or PT license in the state of employment.
  • Participate in in‑service program development.
  • Consistently demonstrate core values and maintain a professional appearance as a representative of the company.
  • Understand, implement, and practice policies and procedures for agency operation.
  • Act as a back‑up Administrator as needed.
  • Arrive at assigned location on scheduled work days and work according to designated hours, including on‑call as needed in office and field nursing responsibilities.
  • Utilize computer and phone continually to maintain constant contact with field staff, physicians, referral sources, internal staff and other departments.
  • Maintain knowledge of ASHN billing, authorization, intake, clinical review, and workflow processes.
  • Completely and correctly submit all required documentation within the specified company requirements.
  • Other duties as assigned.
Management Responsibilities
  • Responsible for day‑to‑day management of field staff.
  • Interview, hire, train, and terminate department personnel.
  • Provide consistent coaching and challenge opportunities to direct reports.
  • Supervise direct reports with consistency and clarity of expectations.
  • Complete yearly evaluations accurately and in a timely manner.
  • Perform coaching, counseling, and discipline as needed.
  • Work with direct reports to ensure employee satisfaction and resolution of issues.
  • Manage time‑card administration.
Qualifications
  • Minimum of three (3) years of experience as an RN or PT in an acute care setting.
  • Strong critical‑thinking skills with the ability to problem‑solve clinical needs.
  • Ability to create positive impressions and communicate with a variety of people.
  • Exemplary communication skills and customer focus.
  • Ability to remain calm, patient and accommodating.
