RN, Healthcare Nursing, RN Nurse

QUALIFICATIONS

1. Graduate of an accredited nursing program with a current license (in good standing (as a Registered Nurse in the states) of practice.

2. Knowledge and ability to provide skilled nursing care as defined in the state nurse practice act.

3. Current CPR certification.

4. One (1) Year home care experience but willing to train the right candidate.

5. Has valid driver's license and use of an insured auto or access to adequate transportation.

6. Excellent verbal and written communication skills, and strong interpersonal skills.

JOB DESCRIPTION

1. Assessing clients' physical and mental wellness, needs, preferences and abilities and using these to develop tailored care plans

2. Working with clients, family and friend support networks and health care professionals to put care plans in place

3. Listening to clients' concerns and providing counseling or intervention as required

4. Recording clients' progress including charting referrals, home visits and other notable interactions

5. Evaluating clients' progress periodically and making adjustments as needed to improve outcomes
