• Provide direct patient care, including wound care, medication administration, and vital signs monitoring
• Develop and implement individualized care plans in collaboration with the healthcare team
• Document patient progress, observations, and medication administration accurately and timely
  
  Assist with resident activities, such as bathing, dressing, and feeding
• Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
• Maintain a clean, safe, and organized environment in accordance with regulatory standards

• Current licensure as a Licensed Practical Nurse (LPN) in the state of Ohio
• High school diploma or equivalent required; associate's degree or higher in nursing preferred
• Minimum of 2 years of experience in long-term care or related field
• Ability to lift, transfer, and reposition patients; standing, walking, and bending required
