Licensed Practical Nurse (LPN) – Home Care

Schedule:

8:15am – 5:00pm, Monday–Friday, 1 weekend & 2 weeknights/month on call

We care for patients like family. As an LPN you will provide skilled nursing care in patients’ homes, working autonomously and collaborating with physicians, case managers, and family members.

Major Areas of Responsibility
  • Plan of Care:
    Provide skilled nursing care as ordered by the attending physician, monitor patient conditions, and notify the case manager of any changes.
  • Minimize Patient Risk:
    Administer medications as prescribed, educate patients on preventive measures, and reduce rehospitalizations.
  • Family Educator/Advocate:
    Educate patients and families on disease processes using teach‑back methods to ensure understanding.
  • Policies:
    Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
  • Rules & Regulations:
    Follow agency policies, procedures, rules, and regulations; communicate schedule changes to schedulers or supervisors.
  • Operations:
    Attend in‑service training and mandatory agency meetings.

Requirements
  • Licensed Practical Nurse (LPN) with current license in the state of employment
  • Minimum one‑year experience as an LPN in an acute care setting; home‑care experience preferred
  • Valid driver’s license and auto insurance in your name as a driver
  • Capacity to meet all physical job demands
