Monday – Friday, 8:15AM – 5:00PM, with on‑call requirements.

Position Summary

The Licensed Practical Nurse (LPN) provides comprehensive skilled care as ordered by the attending physician by following the order sets created by the RNCM within a specified geographical territory. The LPN educates patients and their family members and ensures the safety of the patient. The LPN is accountable for completing accurate documentation and remains compliant with all legal rules and regulations.

The LPN is responsible for all practices and duties within the scope of practice as outlined by the state.

Qualifications & Attributes
  • Licensed Practical Nurse with a current license in the state of employment.
  • Minimum of one year of LPN experience in an acute care setting (two years preferred); home care experience desired.
  • Effective communication skills and the ability to create positive impressions with patients, families, physicians, and colleagues.
  • Calm, patient, and accommodating behavior with compassion and caring while working with patients.
  • Knowledgeable about nursing best practices and able to make appropriate nursing judgments.
  • Detail‑oriented, observant, disciplined work ethic, ability to prioritize and be timely, and ability to follow directions and work as a team member.
  • Valid driver’s license and auto insurance with the nurse listed as a driver.
Major Areas of Responsibility
  • Provide skilled nursing care in a patient’s home as ordered by the attending physician.
  • Perform care by following order sets and goals made by the RN.
  • Observe and monitor patient conditions and notify the RNCM of any changes.
  • Administer medication as prescribed by the physician.
  • Counsel patient and family on disease/injury processes and how to manage.
  • Incorporate patient and family in the development of the plan of care.
  • Help reduce re‑hospitalizations by teaching patients preventative measures and how to make good decisions.
  • Listen to patient and family members.
  • Complete all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines.
  • Maintain an active LPN license in the state of employment.
  • Communicate with scheduler about any changes outside the normal schedule.
  • Participate in all on‑call requirements and case conferences.
  • Attend in‑service trainings and mandatory agency meetings.
  • Maintain a professional appearance as a representative of the company.
  • Understand and practice agency policies and procedures.
Essential Functions
  • Drive to the patient’s primary location per scheduled visit.
  • Maintain daily attendance at assigned visit locations.
  • Document all aspects of subsequent visits within 24hours to meet Medicare guidelines.
  • Complete and submit all required documentation within specified company requirements.
  • Provide skilled care within the scope of practice for a Licensed Practical Nurse.
  • Fulfill visits in the assigned geographic location per patient need within federal and state specifications.
  • Compensation is a lump‑sum payment for all activities connected with a visit to a patient, including preparation time, travel, lab drops, pickup of supplies, charting, and communication with the patient’s family, physician, and other health care providers.
Health Qualifications

Health Requirements:
Must be able to meet all physical position requirements. Evidence of a current negative tuberculin skin test or X‑ray or TB testing on the date of orientation is required. An annual tuberculin skin test is required thereafter, unless the employee has documentation of a prior TB test. Employees must be free of physical/medical conditions that would limit or restrict their ability to perform the job functions listed below.
