Registered Nurse (RN) Clinical Field Staff Liaison - Weekend

Schedule:

Saturday and Sunday: work 8am-5pm and then on call triage from 5p-830am. HOLIDAYS ARE ANY HOLIDAY THAT FALL ON WORKING SHIFT Agency:

SUMMARY:

The RN Clinical Field Staff Liaison (CFSL) Evening/Weekend provides support and acts as a resource to clinicians in the field. The CFSL assists in monitoring compliance quality and safe delivery of services by following a clinical process set by clinical management. The CFSL is responsible for all practices and duties within the scope of practice as outlined by the state.

KEY RESPONSIBILITIES:

Responsible for covering workflow and supervising patient care needs during evening/weekend shifts and provides patient care, per agency needs.  Triage incoming agency calls after hours/weekends and when warranted, will write orders to add on call visits, schedule to appropriate on call staff and communicate with following physicians.  Monitors evening/weekend assignments to align with agency census and potential after hours visits.  Serve as a resource for field staff, referral sources, physicians, patients and their families pertaining to clinical issues and/or concerns during evening/weekend hours.  

Facilitate calls to and from Field staff; uses resources to resolve issues and reports anything of concern to the physician and manager  Work in the field with staff and patients as needed  Maintains positive working relationships with referral sources including physicians, hospitals, skilled facilities, assisted livings and families  Ensure Start of Care compliance  Ensure quality and safe delivery of home health services  Monitor Home Health Aide supervisory visits  Focus on Outcome Monitors as assigned  Implement quality improvement programs in the field  Participates in managing cost of good working with monitoring supply costs and visits per episode  Knowledgeable of Utilization Management workflow and completes timely.  

Review relevant reports: including next day high priority visits for assignment.  Reviews clinical documentation in accordance with agency protocol and Medicare/Federal guidelines  Maintains active RN license in state of employment  Participates in in-service program development  Maintains a professional appearance as a representative of the company  Understands, implements and practices policies and procedures for agency operation  Other duties as assigned

QUALIFICATIONS:

Three years of experience as an RN in an acute care setting  Minimum one-year home health experience is preferred  Ability to effectively communicate and create positive impressions with patients, families, physicians and co-workers  Ability to remain calm, have patience and be accommodating. Compassionate and caring while working with patients  Knowledgeable on nursing best practices and the scope of practice for a nurse as outlined in Chapter 4723 ORC for its RN personnel  Ability to make appropriate nursing judgments  Ability to identify a situation and handle it with the best possible solution  Detail-oriented and observant  Disciplined style of work ethic with the ability to prioritize and be timely  Ability to follow directions and work as a team member  Knowledge of MS Office applications including Word, Excel, PowerPoint and Outlook  Valid driver’s license and auto insurance with your name as a listed driver    EDUCATION AND CREDENTIALS:  Registered Nurse with current license in the state of employment.

Compact licensure, for states that participate, is required to be obtained within first 30 days of employment for additional support in all ASHN affiliated agencies.
