Full Time – Staff

Provides home health visits across Summit County, performing wound care and medication administration, monitoring client condition and following care plans. Requires recent Med/Surg experience and strong wound‑care skills.

• Follow designated care plan in accordance with patient’s needs.
• Make home visits to clients within designated geographic territories.
• Perform assigned duties, including medication administration, wound care, treatments, and procedures.
• Monitor clients’ conditions and report changes to Clinical or Client Services Manager.
• Execute and properly document doctors’ orders.
• Perform client assessments as necessary.
• Case management and coordination.
• Accurately document observations, interventions, and evaluations pertaining to client care management and services, using a touch‑pad tablet.

• Current license as a Registered Nurse in OH.
• Minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care experience strongly preferred.
• Graduation from accredited/approved nursing program, with transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; EMR experience preferred.
• Ability to travel to cases as assigned.
