Registered Nurse (RN) Clinical Field Staff Liaison - Hospice
Schedule:
Monday‑Friday 8a‑4:30p. Nights, weekend and holidays– Administrator On Call rotation.
The CFSL provides support for personnel and patient care services. Responsibilities include reviewing compliance, quality and safe delivery of services by following clinical processes. The CFSL is responsible for ensuring adherence to the Nursing scope of practice as outlined by the State for licensed nursing personnel. Provides backup to the Clinical Manager and Program Assistants as needed.
Qualifications & Attributes- Current RN license in the state of employment
- At least 3years experience as an RN, with at least one year in Hospice
- Strong critical thinking, problem solving and clinical judgment skills
- Excellent communication skills and customer focus
- Calm, patient, accommodating; compassionate and caring with patients, families, and others
- Ability to resolve conflict using conflict management skills
- Knowledgeable in nursing best practices and able to make appropriate judgments
- Detail‑oriented, observant, able to synthesize complex medical information and raise issues to Clinical Management
- Prioritize and be timely, use resources efficiently
- Follow directions, work as part of a team
- Valid driver’s license and auto insurance
- Ensure quality and safe delivery of hospice services for all patients from admission to discharge
- Collaborate with Agency Clinical Manager and Medical Director to facilitate hospice care and ensure compliance
- Meet and exceed quality indicators
- Collaborate with intake, providing daily support and managing assignment volume based on staffing availability
- Complete RN Clinical Manager workflow tasks as needed, identify education needs for field clinicians, and communicate escalation to Clinical Managers
- Focus on key performance indicators as assigned
- Assist with implementation of quality improvement programs in the field
- Participate in on‑call rotation and patient staffing as needed to manage staffing and patient issues
- Complete field visits as required
- Assist in managing cost of goods by monitoring supply, DME, and medication costs
- Maintain positive relationships with referral sources, including physicians, hospitals, skilled facilities, assisted living, and families
- Participate in monthly IDG meetings when needed
- Review clinical documentation in accordance with agency protocol and Medicare/Federal guidelines
- Maintain an active RN license in the state of employment
- Participate in in‑service program development
- Consistently demonstrate core values and maintain a professional appearance as a company representative
- Understand, implement, and practice policies and procedures for agency operation
- Assist in new hire orientation when needed
- Perform additional duties as assigned
