Hospice, General Inpatient Nurse; RN Case Manager

Schedule

Monday - Thursday; 9:00am - 7:00pm; one rotating holiday per year.

• Provide end‑of‑life care and Hospice services in the Health System Inpatient Hospice setting in accordance with standards of practice.
• Assist in determining the need to change the level of care based on assessment and collaboration with the Attending Physician or Medical Director.
• Deliver care in a holistic, family‑centered manner, anticipating, preventing, and treating undesirable or secondary symptoms.

• Bachelor's Degree in Nursing preferred, or Graduate of Diploma or Associate Degree program in Nursing within the state of employment.
• Minimum 3 years inpatient clinical experience, preferably hospice.
• Valid driver's license and auto insurance in your name as a driver.
• Capable of all physical demands.
