• Lead a multidisciplinary team of LPNs, aides, social workers, and chaplains in creating personalized care experiences.
• Advocate for patient needs, preferences, and wishes while supporting families.
• Provide counseling and emotional support to families and patients.
• Develop and implement care plans prioritizing quality of life and comfort.
• Document patient progress and care journeys.
• Provide care throughout the Akron area in homes, assisted living, long‑term care, and hospitals.

• Active RN license
• Valid driver’s license
• At least 1 year of hospice experience

• BLS certification
• Background in hospice and palliative medicine
