As a Home Health LPN, you’ll provide one-on-one skilled nursing care, build meaningful relationships with patients and families, and work alongside a supportive interdisciplinary team focused on positive patient outcomes.

** What You’ll Do
*** Provide skilled nursing care in patients’ homes
* Follow physician orders and care plans created by the Case Manager
* Monitor patient conditions and report changes
* Administer medications and treatments as prescribed
* Educate patients and families using teach-back methods
* Help reduce hospital readmissions through patient education and preventative care
* Complete clinical documentation in accordance with agency and Medicare guidelines
* Communicate schedule or availability changes with the appropriate team members
** Qualifications
* ** Current Licensed Practical Nurse (LPN) license
* Minimum of 1 year of LPN experience in an acute care setting
* Home health experience preferred
* Valid driver’s license and active auto insurance
* Strong communication, time management, and documentation skills
* Compassionate, dependable, and patient-focused
