About This Role

The RN Hospice Case Manager provides care to patients in the Akron/Canton area, focusing on comfort and quality of life for hospice patients and their families. This role is ideal for compassionate caregivers who excel in providing direct patient care and support.

What You'll Do
  • Provide supervision, assessment, planning, and direct patient care to hospice patients.
  • Perform accurate and complete admissions assessments and re-assessments.
  • Assess patients for safety risks, abuse, and neglect.
  • Assess, document, and manage patient’s pain.
  • Prioritize and document patient problems.
  • Administer medications and treatments safely and accurately with a focus on patient comfort.
  • Coordinate care for managing a caseload of patients.
  • Document changes in patient condition and effectiveness and timeliness of care/interventions.
  • Evaluate, update, and revise plans of care.
  • Assess educational needs of patients, caregivers, and family while considering cultural issues and other special challenges.
Requirements
  • Education:
    • Graduate from an accredited school of nursing.
    • Bachelor of Science in Nursing (BSN) preferred.
  • Experience:
    • One year of current clinical experience as an RN.
    • One year of experience in an acute medical-surgical or critical care setting.
    • Two years of critical care and/or med/surg experience preferred.
    • Hospice and/or oncology experience preferred.
  • Licenses &

    Certifications:

    • Current licensure as a Registered Nurse (RN) in the State of Ohio.
    • Basic Life Support (BLS) certification from the American Heart Association (AHA) or the American Red Cross.
    • Valid state driver’s license.
    • Proof of automobile insurance with 100,000/300,000 coverage.
