Registered Nurse; RN), Mobile MDS Coordinator

Communi Care Health Services is seeking a registered nurse (RN) for the Mobile MDS Coordinator position covering multiple skilled nursing facilities across the Midwest Division. The candidate must possess a valid RN license in Ohio and prior long‑term care and MDS experience. Preference will be given to those with multi‑facility or senior MDS experience.

The Mobile MDS Coordinator will be detail‑oriented, collaborative, and capable of communicating effectively with residents, families, and staff at all levels. The role involves traveling throughout Ohio and intermittently to Indiana to work directly in facilities, providing support and ensuring compliance with state and federal regulations. Mileage and travel expenses are reimbursed.

• Evaluate facility compliance with federal regulations and utilization targets.
• Conduct weekly visits for intervention, audit, and training.
• Review resident clinical records for coverage, utilization, accuracy, and timely transmission.
• Provide consultation and expertise on federal and state reimbursement systems.

• Current RN license in Ohio.
• At least 2 years experience managing Medicare/Medicaid and Managed Care reimbursement systems, including MDS and RAI processes.
• Senior MDS experience and/or multi‑facility MDS experience preferred.
• AANAC certification highly preferred.
• Excellent written and verbal communication skills.
• Ability to multi‑task and oversee functions at multiple facilities daily.
• Current CPR certification.
