Schedule:
Monday - Friday 8:30 AM–4:30 PM; one weekend day a month and rotating holidays.
- As a Hospice Registered Nurse Case Manager (RNCM), help patients and their families navigate end-of-life care using hospice best-practice approaches and available tools and resources.
- Collaborate with an interdisciplinary team to deliver a unique, patient-centered experience.
- Symptom management: conduct thorough assessments and coordinate with the patient’s attending physician and hospice Medical Director to manage symptoms.
- Plan of Care: evaluate and individualize the plan of care, adjusting as patient needs change.
- Continuity of Care: oversee interdisciplinary care and ensure appropriate admissions, transfers, and discharges.
- Family Educator/Advocate: counsel patient and family on disease progression and involve them in developing the plan of care; listen to patient and family members.
- Documentation: complete all clinical documentation following agency protocols and Medicare/Federal guidelines.
- Operations: supervise total care of patients provided by nurse aides and LPNs; attend in-service trainings and mandatory meetings.
Compassionate communicator with a positive attitude. Patience when working with patients, families, physicians, and coworkers. Attention to detail and ability to follow directions.
Qualifications- Registered Nurse with a current license in the state of employment.
- Minimum one year of RN experience in an acute care setting (preferred).
- Hospice experience is desired.
- Valid driver’s license and auto insurance in your name as a driver.
- Ability to meet physical demands of the role.
