Summa Hospice is seeking a Hospice Registered Nurse Case Manager (RNCM) to help patients and families navigate end-of-life care in the home setting. You will work with an interdisciplinary team to deliver compassionate, evidence-based care and coordinate services with physicians and staff.
Schedule:
