As a Licensed Practical Nurse (LPN), you deliver essential direct patient care, playing a vital role in our healthcare team. You contribute to patient well-being by providing compassionate support and skilled nursing interventions. This role thrives on your ability to assess, monitor, and respond to patient needs, ensuring a safe and comfortable environment. You act as a crucial link between patients, families, and the broader clinical staff, making a tangible difference in the lives of those we serve every day.

Job Duties and Responsibilities
  • Patient Care: Provides comprehensive direct patient care, including assisting with daily living activities, personal hygiene, and mobility. Monitors and observes patient conditions, identifying changes and reporting them promptly to the Registered Nurse or physician.
  • Medication Administration: Administers prescribed medications safely and accurately, observing for adverse reactions and documenting effects.
  • Vital Signs: Takes and records vital signs, interpreting readings and recognizing abnormal results.
  • Wound Care: Performs wound care, dressing changes, and assesses the healing progress of wounds.
  • Catheterization: Assists with or performs catheterization procedures, maintaining sterile technique.
  • Documentation: Accurately documents all patient observations, care provided, and interventions in the Electronic Health Records (EHR) system.
  • Electronic Health Records (EHR): Utilizes EHR systems effectively for charting, retrieving patient information, and communicating with the healthcare team.
  • Communication

    Skills:

    Communicates clearly and empathetically with patients, their families, and all members of the healthcare team.
  • Infection Control: Adheres strictly to infection control policies and procedures to prevent the spread of illness.
  • Basic Life Support (BLS): Maintains current BLS certification and applies life-saving skills as needed in emergency situations.
  • Collaboration: Collaborates with Registered Nurses and other healthcare professionals to implement and evaluate patient care plans.
Required Qualifications Education
  • Graduation from an accredited Licensed Practical Nurse (LPN) program.
  • Current, unencumbered LPN license in the state of practice.
Experience
  • One (1) year of experience as a Licensed Practical Nurse in a clinical setting.
Knowledge/Skills
  • Medication Administration: Demonstrates proficiency in safe and accurate medication administration.
  • Patient Care: Possesses strong patient care skills with a compassionate and patient-centered approach.
  • Vital Signs: Accurately assesses, records, and interprets vital signs.
  • Electronic Health Records (EHR): Proficiently navigates and utilizes Electronic Health Records systems.
  • Wound Care: Competently performs various wound care techniques and assessments.
  • Basic Life Support (BLS): Holds current and valid Basic Life Support (BLS) certification.
  • Communication

    Skills:

    Exhibits excellent verbal and written communication skills.
  • Documentation: Maintains meticulous and accurate documentation practices.
  • Infection Control: Applies comprehensive knowledge of infection control principles and practices.
  • Catheterization: Competently assists with or performs catheterization procedures.
  • Demonstrates critical thinking and problem-solving abilities in clinical situations.
  • Works effectively in a fast-paced and dynamic healthcare environment.
Preferred Qualifications
  • Experience in a specialized area such as geriatrics, acute care, or clinic settings.
  • Additional certifications relevant to the area of practice.
Physical Requirements /Work Conditions
  • Ability to stand, walk, bend, lift, and push for extended periods during shifts.
  • Ability to safely lift and reposition patients with assistance as needed.
  • Works in a clinical environment with potential exposure to bodily fluids, infectious diseases, and medical equipment.
  • Requires manual dexterity for performing patient care tasks, administering medications, and operating equipment.
  • Ability to respond quickly and effectively to emergency situations.
