Home Health RN

Role:

Skilled Nursing Registered Nurse (RN) providing comprehensive, patient-centered care in the home setting.

• Deliver skilled nursing care in compliance with professional, regulatory, and organizational standards.
• Conduct thorough patient assessments and ongoing evaluations to determine needs and guide care plans.
• Develop, implement, evaluate, and revise individualized care plans collaboratively with physicians, patients, families, and the healthcare team.
• Follow physician orders, coordinate with therapy services, community agencies, and other providers.
• Apply clinical judgment and critical thinking to make independent nursing decisions.
• Educate patients, families, and caregivers on disease management, medications, treatments, and health promotion.
• Maintain accurate clinical documentation and communicate patient status effectively.
• Participate in on-call rotations, Quality Improvement activities, staff meetings, and training.

• Proficiency with Microsoft Office; experience with EMR preferred.
• Strong organizational, communication, critical thinking, and time management skills.
• BSN preferred or to be obtained within five years; current Ohio RN licensure.
• BLS certification from AHA required.
• Minimum of one year of in-hospital or home care nursing experience; interdisciplinary collaboration experience preferred.

Full-time, onsite role with 40 hours per week, focusing on delivering safe, effective, and patient-centered care in a home environment.
