About This Role

As an RN Case Manager, you will provide high-quality nursing care to patients in their homes, covering the Tuscarawas County area. This role is ideal for those who are passionate about patient-first healthcare and are looking to build a rewarding career.

• Pay: $73,840.00 - $
• Location: Akron, OH
• Schedule: Full-time, Day shift

• Provide high-quality nursing care to patients in their homes.
• Conduct assessments and complex nursing treatments.
• Educate patients to help avoid readmissions to acute care.
• Participate in a residency core curriculum based on individual learning needs.
• Provide direct nursing care alongside a success coach.
• Establish and/or revise priorities for patient care.
• Respond to data indicating risk to patients’ health and initiate corrective actions.
• Seek supervision, consultation, and assistance when needed.
• Document in patients’ medical records.

Education:

• Graduate from an accredited School of Professional Nursing
• Bachelor’s of Science in Nursing (BSN) preferred

Experience:

• Minimum one year current clinical experience as a registered nurse
• Minimum one year experience in an acute medical-surgical or home care setting

Licenses &

Certifications:

• Current state licensure as a Registered Nurse (RN)
• Basic Life Support (BLS) Certification through the American Heart Association (AHA) or the American Red Cross
• Valid Driver’s License (VDL) in Ohio with proof of $100,000/$300,000 automobile insurance coverage
