We are seeking a compassionate and skilled LPN to join our team of dedicated healthcare professionals.

As an LPN at Meadow Falls of Bath, you will be responsible for providing direct patient care to our residents, under the supervision of our registered nurses. Your duties will include:

• Administering medications, treatments, and procedures as prescribed by physicians
• Providing wound care, taking vital signs, and performing other basic nursing tasks
• Assisting with daily living activities, such as bathing, dressing, and feeding
• Maintaining accurate records and communicating with the healthcare team
• Providing emotional support and comfort to our residents

• Provide high-quality patient care, promoting independence and dignity for all residents
• Work collaboratively with other healthcare professionals to develop and implement individualized care plans
• Maintain accurate and thorough records of patient care and treatments
• Participate in quality improvement initiatives and contribute to a positive and respectful work environment
• Collaborate with the healthcare team to identify and address resident needs and concerns

• LPN licensure in the state of Ohio
• 1 years of experience

• Ability to lift, transfer, and reposition residents with assistance
• Strong communication and interpersonal skills
