Community Resource and Referral Center Social Worker

The Community Resource and Referral Center Social Worker works at a community site where chronically homeless Veterans or Veterans who are at risk of becoming homeless can access immediate services. The role of the CRRC social worker will also be to provide outreach, case management, and follow-up services in the community and the incumbent will be utilizing a government vehicle on a regular basis.

The position duties include, but are not limited to:

• Provides community outreach to Veterans in housing crisis and identifies and screens Veterans for psychosocial needs related to mental health, medical issues and substance abuse and addiction.
• Provides knowledge and ability to implement treatment modalities, provide counseling and/or psychotherapy and case management for the complex needs of Veterans and their families who are homeless or at risk of homelessness.
• Develops treatment plans in collaboration with the Veteran/family and with the interdisciplinary treatment team.
• Coordinate community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs.
• Provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members/community partners as directed by Veteran.
• Ability to evaluate incumbents' own practice through professional peer review, case conferences and other organizational means.
