Job Overview
The Case Manager for the Inpatient Rehabilitation Unit is responsible for the coordination and delivery of services to patients and families/caregivers within the limits of the financial resources of the patient. The Case Manager will act as a liaison to the payor source assuring cost effectiveness of treatment provided by the rehabilitation team; facilitates and coordinates the discharge planning process; serves as a patient and family advocate to coordinate and direct effective communications regarding the patient’s care;
subject to call back as required.
- Licensed Social Worker (Alabama State Board of Social Workers) or Registered Nurse (Alabama State Board of Nursing).
- One to two years of case management/social work experience in acute care, rehabilitation, or workers compensation settings.
- Minimum BLS certification required; must be obtained within 30 days of hire and renewed per BLS guidelines.
- Provides case management services to all assigned patients and their families to ensure appropriate treatment, benefit utilization, and discharge planning.
- Tracks length of stay to ensure timely discharges, initiates appeals processes per policies, and extends length of stay when appropriate.
- Investigates insurance resources availability and maximizes them.
- Functions as a liaison to referral sources and payors to insure effective communication regarding preliminary plan of care, progress toward program goals, and expected discharge plans.
- Coordinates IRF team conference/staffing meetings weekly and reports to external review parties.
- Schedules clients for outside appointments and coordinates medical care as directed by physician, completing appropriate paperwork.
- Maintains weekly contact with families to facilitate involvement, appraise progress, coordinate discharge planning and investigate funding issues.
- Ensures quality, timeliness and confidentiality of medical records documentation.
- Participates in departmental meetings, in‑service education, and professional affiliations to enhance professional growth and stay current with case management practices.
- Communicates with the Business Services Department regarding pre‑certification, continued stay reviews, and additional insurance coverage.
- Negotiates, orders and arranges delivery of services, equipment and supplies during client stay and discharge alone or with therapy services.
- Facilitates and coordinates discharge planning and serves as patient and family advocate.
- Acts as liaison to payor source to assure cost effectiveness of rehabilitation team treatment.
