- Evaluate patients and develop individualized treatment plans.
- Provide skilled physical therapy services to post-acute and skilled nursing residents.
- Improve strength, mobility, balance, endurance, and functional independence.
- Monitor patient progress and adjust treatment plans as needed.
- Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and other healthcare professionals.
- Educate patients and families on treatment goals and home exercise programs.
- Maintain accurate, timely, and compliant clinical documentation.
- Current and unrestricted California Physical Therapist (PT) License required.
- Previous experience in a skilled nursing facility, rehabilitation, or post-acute setting preferred.
- Strong clinical, communication, and time-management skills.
- Ability to work independently while contributing to a team-oriented culture.
- Passion for delivering exceptional patient-centered care.
