Social Worker, Crisis Counselor

Vital Caring seeks a compassionate Social Worker to support home health and hospice patients and families. In this role, you'll complete psychosocial assessments, develop care plans, provide counseling and crisis intervention, and connect patients with community resources. You'll collaborate closely with an interdisciplinary clinical team to support safe transitions, chronic disease management, and end-of-life care. Responsibilities

• Conduct psychosocial assessments for home health and hospice patients
• Develop and update patient-centered care plans in collaboration with the care team
• Provide counseling, crisis intervention, and caregiver support in the home setting
• Coordinate community resources, benefits, and support services for patients and families
• Advocate for patient needs while ensuring regulatory and ethical compliance
• Document care in EHR systems accurately and timely
• Participate in interdisciplinary team meetings and case conferences
• Educate patients and families on illness adjustment, coping, and advance care planning
