As an Admissions Nurse (RN), you are the first clinical impression families and residents experience. Your skill, compassion, and professionalism set the tone for a smooth transition into our community.

We are seeking a skilled and compassionate Admissions Nurse (RN) to join our team. This role plays a key part in welcoming new residents, coordinating admissions, and ensuring a seamless transition into our facility.

This position requires strong clinical assessment skills, attention to detail, and a resident-centered approach.

• Conduct initial clinical assessments of new residents
• Develop individualized care plans based on assessment findings
• Coordinate with families, physicians, hospitals, and other healthcare providers
• Ensure all required documentation is completed accurately and timely
• Educate residents and families on services, policies, and care processes
• Collaborate with the interdisciplinary team for continuity of care
• Provide emotional support during the transition process
• Participate in quality improvement initiatives related to admissions

• Current Registered Nurse (RN) license in good standing
• 1–2 years of nursing experience preferred (long‑term care or geriatrics a plus)
• Strong assessment and documentation skills
• Knowledge of state and federal nursing home regulations
• Excellent communication and interpersonal skills
• Ability to work independently and collaboratively
• Compassionate, patient‑focused approach
