Licensed Practical Nurse (LPN)

Sumter Health & Rehabilitation

Shift: 7:00 PM – 7:00 AM (12-Hour Night Shift)

Compensation

Base Pay: $27.05–$29.55 per hour (based on verified LPN experience)

Additional Shift Differentials
  • +$3.00/hour for all hours worked on the 7:00 PM–7:00 AM night shift
  • +$3.00/hour for 7:00 AM–7:00 PM weekend day shifts
Example

An LPN with a base pay rate of $28.50/hour would earn $31.50/hour while working the 7:00 PM–7:00 AM night shift.

Position Summary

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our interdisciplinary team. The LPN will provide exceptional nursing care while working closely with physicians, registered nurses, therapists, and other healthcare professionals to ensure the highest quality care for our residents.

Key Responsibilities
  • Provide direct nursing care to residents in a long-term care and rehabilitation setting.
  • Assess, monitor, and document residents' health status and report changes appropriately.
  • Administer medications and treatments as prescribed.
  • Assist in developing and implementing individualized care plans.
  • Maintain accurate and timely documentation in accordance with state and federal regulations.
  • Promote a safe, comfortable, and therapeutic environment for residents.
  • Supervise and support Certified Nursing Assistants in providing quality resident care.
  • Communicate effectively with residents, families, physicians, and the interdisciplinary care team.
Qualifications
  • Current Licensed Practical Nurse (LPN) license.
  • Strong clinical and assessment skills.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively in a team-oriented environment.
  • Compassionate, resident-focused approach to care.
  • Current BLS/CPR certification required.
