Wound Care / Treatment Nurse – LPN or RN Baylor Weekend Position

Crowne Health Care is currently accepting applications for a Wound Care / Treatment Nurse (LPN or RN) to join our long-term care and rehabilitation team specializing in geriatric care.

This Baylor Weekend position is ideal for a dependable, compassionate nurse who takes pride in providing high-quality wound and treatment care. The successful candidate will demonstrate strong assessment skills, attention to detail, accountability, and a positive, team-oriented attitude while working closely with residents, families, nursing staff, and other members of the interdisciplinary team.

• Current LPN or RN license in the State of Alabama
• Previous wound care or treatment experience preferred
• Wound Care Certification is a plus
• Candidates without wound care certification may be considered;
  the facility is willing to train and cover the cost of certification
• Strong assessment, documentation, and communication skills
• Dependable, organized, and able to work effectively as part of an interdisciplinary team
• Long-term care experience preferred

• Assess residents’ skin integrity, wounds, and treatment needs and provide appropriate follow-up care.
• Perform and document ordered wound care and treatments in accordance with physician orders, facility policies, and applicable regulations.
• Provide treatment for acute and chronic wounds, ostomies, stomas, skin tears, pressure injuries, and skin breakdown related to urinary or fecal incontinence
  .
• Monitor residents for changes in skin condition and promptly communicate concerns to appropriate nursing staff and providers.
• Work closely with licensed nursing staff to identify and address risks such as skin breakdown, contractures, weight loss, and other conditions that may affect residents’ functional status.
• Coordinate treatments with residents’ daily schedules and other departments to promote continuity of care.
• Assist with development, implementation, and follow-up of resident care plans related to wounds, skin integrity, and treatments.
• Maintain accurate and timely documentation in accordance with facility policies and procedures.
• Communicate with physicians, providers, families, nursing staff, therapy, dietary, and other members of the interdisciplinary team as needed.
• Participate in staff meetings, in-services, quality assurance activities, and wound prevention initiatives.
• Provide a multidisciplinary approach to wound management and treatment to promote healing and maintain the highest possible level of resident care.
