The Care Coordination Program Manager ensures that there is program alignment with partner care management plan and ensures coordination between Help Me Grow and ECM services; supervising and guiding CHW staff to deliver high quality services.
- Leads CHW service delivery and ensure compliance with CalAIM requirements.
- Develops and refines operational workflows and processes to support effective CHW operations.
- Coordinates with Managed Care Plan partners to support program alignment and ensure compliance with contractual, quality, and promising practices standards.
- Ensures timely, accurate staff documentation to support billing and reporting requirements.
- Monitors team performance and communicates findings to management; implements improvements to enhance efficiency and service quality; maximizes effectiveness and ensures alignment with department's mission.
- Recommends and implements goals and objectives, policies and procedures and ensures the program is staffed with qualified employees by supporting hiring, training, performance management, and staff development.
- Manages program supervisors by setting expectations, communicating goals, and ensuring quality, timeliness, and policy compliance in daily operations.
- Develops program plans, tools, and trainings to clarify roles and strengthen staff and partner performance.
- Strong understanding of CalAIM, Community Health Worker (CHW) Benefit, Medi-Cal managed care operations, and experience developing operational workflows.
- Knowledge of Enhanced Care Management (ECM) and Help Me Grow
- Knowledge of early childhood delivery systems, pediatric preventive care and social determinants of health.
- Knowledge in serving families accessing and navigating health care, developmental and behavioral services.
- Principles and practices of employee supervision, including work planning, assignment review and evaluation, discipline, and the training of staff in work procedures.
- Principles and practices of leadership.
- Principles and techniques for working with groups and fostering effective team interaction to ensure teamwork is conducted smoothly.
- Principles and practices of program management including planning, development, implementation, and evaluation.
- Modern equipment and communication tools used for work functions and program, project, and task coordination, including computers and software programs relevant to work performed.
- Ability to manage programs, implement service models, and ensure compliance with contractual and regulatory requirements.
- Excellent communication, facilitation, and partnership-building skills.
- Experience collaborating with managed care plans, community partners, and cross-functional teams
- Ability to use data for program monitoring, quality improvement, and reporting.
- Ability to guide teams through change in a fast environment; leadership, problem-solving and adaptability skills
- Develop and implement goals, objectives, practices, policies, procedures, and work standards.
- Provide administrative, management, and professional leadership for the assigned program.
- Select and supervise staff, provide training and development opportunities, ensure work is performed effectively, and evaluate performance in an objective and positive manner.
- Ensure the maintenance and retention of complex records, files, reports, and other documents.
- Independently organize work, set priorities, meet critical deadlines, and follow-up on assignments.
- Effectively use computer systems, software applications relevant to work performed, and modern business equipment to perform a variety of work tasks.
- We are currently working a hybrid 2 day per week in-office schedule and employees also come to the office to attend meetings and as requested.
- All First 5 employees must live and work in California.
- The salary range for this position is $99,000 to $142,000 and as an independent government agency, the posted salary is the set salary range for this position. We cannot offer salaries above this posted range.
