Under the supervision of the Clinical Manager, this role involves delivering appropriate, adequate, effective, and efficient direct patient care services as assigned by the Primary Care Clinician and approved by the Clinical Manager.

• Implement the physician's plan of care.
• Obtain physician orders.
• Supervise home health aides.
• Coordinate care within an interdisciplinary team.
• Participate in effective utilization management and control of patient supplies.

Consistently provide care based on the age-related needs of the unique population, considering their developmental and psychosocial needs, environmental safety, educational needs, communication style, and level of understanding.

This position is accountable to the Primary Care Clinician for ensuring that the plan of care is implemented following professional standards of care and agency-specific protocols.
