Summary

The AHD Registered Nurse
- Park Bridge provides direct nursing care to residents and supervises the patient care performed by certified nursing assistants (CNAs) and licensed vocational nurses (LVNs). Supervision and care are in accordance with current federal, state, and local standards, guidelines, and regulations governing the facility. The facility operates 24/7; hours and days worked may vary. Performs related duties as required.

Duties & Essential Job Functions

Uses a systematic approach to provide individualized nursing care. Establishes priorities of patient care based on essential needs and available resources.

Coordinates admission, discharge, and transfer of Residents. Conducts resident rounds every two hours to assess status and initiate interventions. Documents symptoms, responses, and status. Collects data for assessments and care plans; records data accurately and timely.

Makes nursing diagnoses to guide care strategy. Institutes appropriate interventions to stabilize conditions and prevent complications; evaluates responses to interventions.

Consults with physician or licensed independent practitioner. Clarifies orders if needed and notifies when decisions affect medications or treatments. Understands rationales for medications and treatments and administers them correctly.

Reviews medication and treatment records for completeness and accuracy. Administers and documents medications, enteral nutrition, IV therapy, and treatments per physician orders and policies. Orders and receives medications as appropriate.

Designs, promotes, and coordinates Resident education and counseling. Collaborates with the Resident and healthcare team; initiates referrals to community resources to ensure continuity of care.

Plans and coordinates nursing rehabilitation/restorative care. Initiates interventions to promote independence and documents care plans and records.

Supervises nursing staff in rehabilitation/restorative activities and trains CNAs in related techniques and prevention measures.

Coordinates nursing interventions with therapies (PT, OT, ST, RT). Communicates status to staff and family; documents concerns and works toward resolution; addresses grievances as appropriate.

Minimum Qualifications

Education:

Graduate of an accredited Nursing program required.
Education:

Bachelor's degree preferred.
Experience:

Minimum of one year in a direct care role; new graduates considered.
Licenses/Certifications:

Valid Registered Nurse license in California.
Licenses/Certifications:

Current BLS – Basic Life Support certification.
