Description Position Summary Manage a complex caseload while providing intensive client support, advocacy, and coordination of services.
Responsibilities- Manage high-volume caseloads.
- Develop and implement individualized service plans.
- Coordinate with external agencies and stakeholders.
- Ensure compliance with program requirements.
- Prepare reports and maintain documentation.
- 3+ years of case management experience.
- Knowledge of community-based resources.
- Strong crisis intervention and problem-solving skills.
- Experience with case management databases.
