Overview

Salary Range $77,000.00 - $ Salary Category Health Care

Description

Under general supervision, provides direct service to Alameda County families providing information and linking families to services and community resources that support and/or enhance their child(ren)'s development; assesses needs and eligibility for programs/services, processes referrals and inquiries from the early childhood central access point, and manages assigned cases. Additionally, as agency and program needs evolve, additional duties may be assigned to support service delivery and departmental priorities

  • Processes a high volume of referrals and inquiries received by the Help Me Grow (HMG) Phone Line/ early childhood central access point in support of early childhood development and social-emotional well-being.
  • Contacts families via telephone to address developmental and behavioral questions or concerns and other family needs; develops child-centered care plans, assists with understanding and supporting their child's development, understanding available resources and services, and empowers and educates families to navigate resources and advocate for their child(ren).
  • Assesses child/family needs and eligibility for programs/services; identifies appropriate referrals and coordinates referral to community resources, entitlement systems, and services that address social determinants of health; links families to other HMG components such as family navigation, and/or other agencies for family support and/or further assessment.
  • Consults with service providers regarding potential referrals, developmental and behavioral concerns of children, appropriate referral pathways, and community resources available to families.
  • Conducts timely follow-up with families and providers to monitor developmental progress, ensure connection with services, and provide additional information and activities; provides feedback to referral sources of status and outcomes.
  • Reviews and maintains accuracy of data of HMG Phone Line activities; analyzes data and makes recommendations for improvement; identifies systems barriers to service delivery and access and collaborates with HMG staff and partners to address barriers.
  • Cultivates and maintains strong interagency relationships with community agencies/resources in order to ensure coordinated linkages to needed services.
Qualifications

Minimum Qualifications

Any combination of training and experience that would provide the required knowledge, skills, and abilities is qualifying. Additional experience and/or education can be substituted to meet typical qualifications. A typical way to obtain the required qualifications would be:

Three (3) years of increasingly responsible experience in child development, social service, or psychology supplemented by college level coursework or training in child development, social work, psychology, or a closely related field.

Additional relevant work experience may be substituted to meet the college coursework/bachelor's degree standard.

Knowledge of:

  • General knowledge of child development (typical and atypical), developmental disabilities, infant and early childhood mental health, family support, and the social determinants of health.
  • Principles and practices of assessment and early intervention of children birth to age five.
  • Social service, health, education and developmental agencies and resources in Alameda County.
  • Entitlement systems for children birth to age five and their families, including eligibility requirements.
  • Outreach, interviewing, and care coordination/navigation skills and techniques.
  • Strategies for effective problem-solving and identification of systems issues.
