Licensed Practical Nurse

San Luis Care Center in San Luis, CO is seeking a Licensed Practical Nurse to provide compassionate, resident-centered care in our skilled nursing and rehabilitation facility. The LPN will deliver direct bedside care, administer medications and treatments, monitor vital signs and resident conditions, and document accurately in medical records. The role collaborates closely with RNs, therapists, and CNAs to support care plans, rehabilitation goals, and safe transitions.

Responsibilities

• Provide direct bedside nursing care to residents in a skilled nursing and rehab setting
• Administer medications and treatments per physician orders and facility protocols
• Monitor vital signs, symptoms, and resident conditions, reporting changes promptly
• Document care, observations, and interventions accurately in medical records
• Support and implement individualized care plans and rehabilitation goals
• Collaborate with RNs, therapists, CNAs, and physicians to coordinate care
• Educate residents and families on care, treatments, and health maintenance
• Promote resident dignity, safety, and comfort in all interactions
• Assist with admissions, discharges, and transitions of care
• Maintain infection control, safety, and regulatory compliance standards
