Minimum 1+ years pharmacy experience required.
RPh on the Go is seeking a dedicated and experienced Staff Pharmacist to join our team at a Long Term Care facility in Wilkes-Barre, Pennsylvania. This is a full-time, day shift position that offers a unique opportunity to make a significant impact on the health and well-being of residents in a supportive and collaborative environment.
Key Responsibilities- Provide comprehensive pharmaceutical care to residents in a Long Term Care setting.
- Review and verify medication orders for accuracy and appropriateness.
- Collaborate with healthcare providers, nurses, and other staff to ensure optimal patient outcomes.
- Monitor and manage medication therapy, including adjustments based on patient-specific factors.
- Ensure compliance with state and federal regulations, as well as facility policies.
- Maintain accurate records and documentation in accordance with regulatory standards.
- Provide patient education and counseling as needed.
- Monday - Friday: 8:00am - 5:30pm
- Rotating weekends.
- Pharmacy experience
- LTC experience a plus
- PA pharmacy license
