About the Role

The Community Health Aide plays a vital role in improving public health outcomes by providing direct support and education to diverse communities. This position focuses on bridging gaps between healthcare providers and community members, ensuring access to essential health services and resources. The role involves conducting outreach, facilitating health screenings, and promoting preventive care initiatives tailored to the unique needs of the population.

The Community Health Aide collaborates closely with healthcare professionals, local organizations, and government agencies to implement effective health programs. Ultimately, this position aims to empower individuals and families to make informed health decisions, contributing to the overall well‑being and resilience of the community.

Minimum Qualifications
  • High school diploma or equivalent required or TABE Level D (8th‑grade reading & math)
  • Experience working in community health, social services, or a related environment.
  • Basic knowledge of public health principles and community outreach strategies.
  • Strong communication and interpersonal skills to engage effectively with diverse populations.
  • Ability to work independently and as part of a multidisciplinary team.
  • Resides in assigned community or sub region.
  • Able to provide year‑round coverage of clinic hours.
Preferred Qualifications
  • Certification as a Community Health Worker (CHW) or similar credential.
  • Associate degree in health sciences or related field.
  • Experience working with underserved or vulnerable populations.
  • Proficiency in multiple languages commonly spoken in the community served.
  • Familiarity with electronic health records (EHR) and data collection tools.
  • Training in cultural competency and health education methodologies.
Responsibilities
  • Conduct community outreach to educate residents about available health services and preventive care.
  • Assist in organizing and facilitating health screenings, immunization drives, and wellness workshops.
  • Serve as a liaison between healthcare providers and community members to improve communication and service delivery.
  • Collect and report data on community health trends to support program development and evaluation.
  • Provide culturally sensitive support and guidance to individuals seeking healthcare resources.
  • Collaborate with local organizations and public health agencies to coordinate health promotion activities.
  • Maintain accurate records of outreach activities and participant interactions.
  • Identify barriers to healthcare access within the community and propose actionable solutions.
