Personal Care Attendant in Stebbins
Purpose of Position:
Provide assistance to consumers of the In Home Support/PCA Program. Enable consumers to obtain or maintain an independent living situation that allows them the least restrictive environment by helping them remain in the region and in their own homes.

Essential Functions:
- Provide outreach services as a primary worker with clients, parents and families, becoming completely familiar with consumer’s Individualized Service Plan (ISP) and implementing assigned duties from that plan.
- Promote learning about social skills, boundaries, living skills and hygiene, assisting the development and promotion of community awareness programs.
- Provide personal care as noted in the consumer’s ISP, including bathing, dressing, transferring, etc. Provide chore service as noted in the consumer’s ISP, including light housekeeping tasks, laundry, shopping, etc.
- Maintain daily reporting through case notes and Medicaid documentation.
- Participate in treatment team meetings when necessary and assist in planning and implementation of recreational, educational, and vocational activities.
- Motivate and encourage consumers to complete required treatment activities by developing positive empathetic relationships with consumers.
- Provide crisis intervention as needed.
- Coordinate with community professionals as required, including NSHC personnel, and personnel from other agencies, etc.
- Attend pertinent training and departmental meetings as requested. Participate in activities that promote professional growth and self-development.
- Perform other duties as assigned.
Required Knowledge:
- Knowledge of patient care principles, practices, standards and techniques, including proper lifting, assistance with walking, etc.
- Knowledge of documentation requirements for specific fields/records.

High School Diploma or Equivalent
Experience- 0 year(s) in a human service or healthcare setting providing direct consumer care
- Must be at least 18 years of age
Driver’s license preferred.
Required training upon hire: BLS or CPR, First Aid Certification, Assistance with Self-Administration of Medication;
Critical Incidents Reports, Mandatory Reporting, Restrictive Intervention, Food Worker’s Card.
Required upon hire: register with AK Background Check.
Unit; apply for NPI, certify with Conduent as Medicaid Provider
