Job Overview
Under the direction of the RN or Rehab Therapist, the Senior Home Services Assistant recognizes and completes patient care needs with minimal direct supervision and performs various basic patient care activities and related services necessary in caring for the personal needs, physical needs, and comfort of patient in the home setting at the level specified in the plan of treatment.

Shifts:
Full-time, 40 hours/week, scheduled visits.
Schedule:
Monday – Friday, days only, no nights or weekends. Holidays required when needed.
Pay:
Starting at $18.94 per hour;
Senior Level $19.50+;
Pay per visit (after training) – Home Services Aide $25.69 per visit, Senior $26.20 per visit.

- Recognizes and completes patient care needs with minimal direct supervision.
- Performs basic patient care activities: toileting, bathing, linen and clothing changes, fresh water and ice, ambulating, comfort measures.
- Coordinates patient schedule, maintains accurate records of time and mileage, and ensures timely visits.
- Follows aide delegation plan, performs duties within scope of practice, and performs specialized procedures with skill validation and regulatory standards.
- Fosters a safe environment by complying with safety policies (infection control, lifting & transferring, disaster preparedness, etc.) and takes appropriate action in emergencies.
- Communicates effectively with patients, reports significant care concerns and safe environment issues to the Case Manager/Agency.
- Observes, records, and reports responses to the treatment plan through point-of-care charting.
- Perform bowel and bladder care including bladder irrigation, bowel stimulation, and digital stool removal.
- Assist patients in self-administering medications and provide simple wound care, ostomy care.
- Assist patients during end-of-life care and medically complex patients who require additional support (quadriplegic, paraplegic, etc.).
- Maintain accurate time and mileage records; keep accessibility during assigned schedule.
- Comply with safety policies and perform specialized procedures with skill validation.
Minimum Qualifications
- Certified Nursing Assistant (CNA) certification.
- BLS for Healthcare Providers certification.
- Current driver’s license, insurance, reliable transportation and acceptable driving record.
- Effective verbal and written communication skills; must be 18 years or older.
- Experience as a CNA or other applicable healthcare experience.
- Experience working in home health or hospice.
- Experience with bowel and bladder care.
- Must be able to see and read information, assess patient needs, operate monitors, and identify equipment and supplies.
- Frequent verbal communication and quick understanding in emergency situations.
- Manual dexterity to manipulate equipment precisely, including computer use for documentation.
- Ability to lift, transfer patients, move supplies weighing up to heavy items, and assist with ambulation.
- Capacity to stand in a stationary position for extended periods; navigate crowded rooms.
- For driving roles, ability to drive safely.
