BAYADA Home Health Care is seeking an experienced Physical Therapist (PT) for a per diem weekend position, performing home health visits in Lehigh and Northampton Counties, PA. One year of licensed PT experience is required.
You will work as part of a multidisciplinary team in clients' homes to provide rehabilitative care and skilled nursing services. Responsibilities include home visits, assessments, care plan development, patient and family education, and EMR documentation.
