Physical Therapist

Baptist Home Health Care by BAYADA is looking for a compassionate and dedicated Full Time Physical Therapist to join our team in Orange Park, FL
. This office serves adult and geriatric clients on a per visit basis. Baptist Home Health by BAYADA offers full‑time schedules.

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements, such as mobility/range of joints, transfer status, stability, ambulation patterns, strength and endurance of muscles, balance testing, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence‑based dressings and topical agents.
• Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continuously assess and revise the Physical Therapy care plan, and collaborate with nursing in multidisciplinary care planning as appropriate.
• Educate and instruct clients, family members, or other representatives in rehabilitative care and activities necessary to promote client health, safety, and independent living.
• Accurately document observations, interventions, and evaluations related to client care using a touch‑pad tablet.

• Current Florida Physical Therapist license.
• Minimum one year of recent work experience as a Physical Therapist.
• Graduated from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem‑solving skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Florida Background Screening Clearinghouse ().
