Job Description
Provides direct personal care and companion supports for individuals and ensures their health, safety, and welfare; ensures compliance with regulations; transports individuals to and from day programs, community outings and other activities, as necessary.
- Reports to the Program Specialist, Managers, and or the CEO.
- Ensures EIG Services mission is incorporated in the provision of individual support.
- Ensures compliance and implementation of EIG Services' policies and procedures.
- Completes tasks and assignments with accuracy.
- Document individual records data/information entry and initialing upon completion.
- Documents all correspondence.
- Provide meals and snacks during certain shifts Document all pertinent information in the Staff binder and review the Previous T-Log documentation at the beginning of each shift on the online platform:
Therap. - Document information pertaining to relevant medical issues, such as injury, illness, and medication changes and observed side effects of medication and a follow-up with an email to Program Specialist or Managers.
- Read and become familiar with the person's ISP for pertinent information [i.e., food allergies, behavioral issues, emergency contact #, etc.)
- Ensures all prescribed medication(s) is dispensed and documented per the policies and procedures in cooperation with the individual's medical needs.
- Ensure that each individual medical needs met by medical providers.
- Documentation and the following instructions given per consults/ consultant.
- Obtaining receipts for purchases made by the individual and for the agency.
- Assists with the maintenance and upkeep of the home.
- Communicates and resolves program and individual concerns with Program Specialist and or Managers on a regular basis.
- Participates in mandatory staff meetings and training.
- Serves as a model for the appropriate language, dress, and behavior.
- Adheres to the appropriate procedures in the event of a crisis situation; complies with instructions given by the Program Specialist and or Managers contacts the appropriate people (both professionals and those important to the individual).
- Ensures effective communication with individual(s), families, and other agencies involved in the support of the individual.
- Participates in various team projects and work groups, as assigned.
- Receives 24 hours of training relevant to Human Services.
- Transports individuals, as assigned, in a safe and timely manner.
- Accompanies individuals on recreational and shopping trips.
- Engages individuals living in the residential site with leisure activities, such as arts & crafts, and games.
- Provides general assistance to the individuals with implementing their activities of daily living.
Must have/be:
- Current Physical with TB testing
- Clean DMV Drivers Vehicle History Check
- 18 years or older
