  • POSITION SUMMARY The Substance Abuse Treatment (SAT) Case Manager will coordinate services for adults at Maniilaq Counseling and Recovery Center. The Case Manager is responsible for ensuring that clients who have been diagnosed with or who are at risk for an Opioid Use Disorder (OUD) are connected with appropriate resources and services, whether through services offered at Maniilaq or those available in the community/state-wide to ensure that consumers in the program have access to all appropriate behavioral health and educational and ancillary services necessary for successful healing and recovery from mental health and substance use.

    They will provide support for the direct service staff in the coordination of various client services, participate in data collection, process and maintains client information using the AKAIMS and or other electronic medical record (EMR) and or billing system, and be responsible for completion of assigned administrative tasks.
  • The Substance Abuse Treatment Case Manager will partner with the substance team and Itinerant Therapists to arrange and design community outreach events and will more generally seek to reduce barriers to clients receiving appropriate treatment.
  • Based on client and agency need, other duties may also be assigned. This position reports directly to the Office Manager.
PRINCIPAL DUTIES AND RESPONSIBILITIES
  • Acts as a liaison between multiple agencies and service providers who are involved with assigned consumers’ treatment.
  • Receives incoming referrals, refers crisis situations to appropriate staff or provider for immediate attention.
  • Provides and processes Intra- and Inter-agency referrals to ensure appropriate and coordinated service provision.
  • Prepares and completes all required documentation in a timely manner.
  • Provides support and advocacy for the consumers and families assigned. Ensures that all referral appointments are kept.
  • Supports BHS staff with weekly consumer data collection, compiles statistics regarding staff, clients, and department activities as needed for reports utilizing basic computer skills.
  • Must be familiar with office equipment including computers, copiers, FAX, and other pieces of office equipment used on a routine basis
  • Provides administrative coverage as needed during vacancies.
  • Appropriately recognizes and maintains the confidentiality of all patient information.
  • Maintains confidentiality of all client, staff, and division related information as per state and federal regulations, Maniilaq Policies, BHS policies and procedures, and staff code of conduct.
  • Participates in community outreach activities and assists clinical staff with outreach as needed.
  • Participate in the delivery of regionally relevant outreach and prevention materials and methods.

  • Provides coordination of care services, ensuring that client receive necessary medical, mental health, and social support as a part of their treatment.
  • Performs a variety of administrative tasks and clerical duties such as filing, correspondence, answering phones, sending quarterly reports, and making copies for staff members.
  • Supports division staff by:
    Sending out meeting notices/reminders, coordinates and attends staff and BHS Advisory committee meetings, processes and distributes minutes.
  • Directly participates in discharge and transitional planning.
  • Prepares monthly and quarterly reports as required by grants and contracts.
  • Communicates with psychiatrist and pharmacy continuum of care.
  • Acts as a liaison between agency, provider, and consumer regarding all medication management related items.
  • Coordinates client travel by communicating and scheduling necessary information with clinical staff and outside agencies.
  • Obtains signatures on appropriate paperwork to meet Federal, State, CARF, and JCAHO requirements regarding patient rights, the Privacy Act, release of information, and advance directives.
  • Promotes good public relations with the community, regional organizations, the State of Alaska, Federal agencies, and MCRC personnel.
  • Possesses good communication skills and an aptitude for working with people. Proficient verbal communication and writing skills.
  • Ability to travel on short notice and in small planes required.
  • Must also be willing to learn multiple computer programs and take on multiple duties and responsibilities
