Job Summary

This position is funded through a three-year State of Alaska Tobacco Control and Prevention grant and therefore has a strong focus on tobacco cessation. This role focuses on health systems change addressing healthcare delivery, patient education and workforce strategies to improve prevention, management of outcomes for chronic conditions related to tobacco such as diabetes, heart disease, hypertension, and cancer. This position bridges clinical care, community outreach, and organization wide change strategies to improve patient outcomes.

This position will work with staff in the Community Wellness team as well as the Joy Janssen Clinic and hospital, under the direct supervision of the Community Wellness and Public Relations Manager.

Responsibilities

The Chronic Disease Prevention Coordinator will collaborate with key departments within PMC to create a supportive environment of care for tobacco cessation and related chronic disease management. This includes the development and implementation of strategies for tobacco use treatment policies and procedures within Petersburg Medical Center that align with current public health guidelines. Additionally, the Coordinator will develop and implement a variety of strategies to support patients at risk for experiencing chronic disease conditions (e.g. hypertension, diabetes).

The State of Alaska Tobacco Prevention and Control (TPC) team provides the resources, guidance, and technical assistance to grantees and this position will serve as the primary contact with the TPC program.

  • Facilitate PMC's Health Systems Change workgroup and leads project planning evaluation effort.
  • Work with healthcare teams to execute quality improvement measures to support tobacco and related chronic disease screenings, referrals, treatment and follow-up.
  • Work collaboratively to identify and support patients at risk for experiencing chronic disease conditions.
  • Monitor progress towards health system change goals and objectives and make recommendations on additional improvements based on outcome data.
  • Develop and oversee media campaigns, patient and staff education resources and tools to promote health and wellness and support patients with chronic conditions.
  • Develop and implement community outreach events and programs that promote healthy behaviors and support self-management skills.
  • Work with leadership to establish staff training opportunities to support project goals.
  • Collect data, analyze findings, and submit progress reports as necessary.
  • Complete required trainings and attend technical assistance calls with the State of Alaska Tobacco Prevention and Control team.
  • Assist with grant management tasks for State of Alaska Tobacco Prevention and Control grant.
  • Assist with planning and implementation of PMC's Employee Wellness Program.
  • Perform other duties as assigned.
