Delta‑T serves a client who needs a Multi‑Site RN Case Manager to support individuals in group homes at an IDD facility in the Northeast Philadelphia area. You will provide case‑management and clinical support to residents through both virtual and in‑person interactions.

8:30 am – 5:00 pm, Monday through Friday.

• Provide case‑management support to individuals in group homes, coordinating care via Zoom and in‑person meetings.
• Conduct clinical assessment and administer medication to patients.
• Visit group homes to deliver medication, update treatment logs, and maintain medical charts.
• Partner with healthcare professionals to coordinate patient care.
• Perform additional duties as assigned.

• Graduate from an accredited nursing program.
• Current RN license in Pennsylvania.
• Valid PA driver’s license.
• Experience or strong interest in working with individuals with physical and developmental disabilities.
• Proficiency with clinical duties such as assessments, treatment, case‑management, and medication administration.
• Experience with Outlook, Microsoft Office, and Word.
