Position OverviewPhoebe Hospice is seeking a compassionate and dedicated Nurse Practitioner to join our interdisciplinary team in Albany, Georgia. This role focuses on delivering high-quality, patient-centered care to individuals facing life-limiting illnesses across Southwest Georgia. The Nurse Practitioner will play a vital role in supporting patients and their families by enhancing quality of life through thoughtful, personalized care and clinical expertise.

Key Responsibilities- Provide comprehensive medical care to patients in accordance with established protocols and standards
- Conduct patient assessments, develop care plans, and collaborate with physicians and the interdisciplinary care team
- Manage and treat symptoms, including pain and other distressing conditions
- Support patients and families through education, communication, and compassionate presence
- Participate in care coordination and case discussions to ensure continuity and quality of care
- Maintain accurate and timely documentation in compliance with regulatory requirements
Master’s degree in nursing. (Required)
Work Experience3 years’ experience in supervision and independent decision making in major nursing areas. (Preferred)
Licenses and CertificationsNP-C - Adult Nurse Practitioner - AANPCP (Required)
BCLS - Basic Life Support (Required)
