Nurse Practitioner, Hospice

Make Every Moment Matter as a Hospice Nurse Practitioner. In this role, you’ll provide comfort, dignity, and expert care to patients and families when they need it most. Phoebe Hospice is seeking a compassionate and dedicated Nurse Practitioner to join our interdisciplinary team in Albany, Georgia, serving patients with life‑limiting illnesses across Southwest Georgia.

• Provide comprehensive medical care to patients in accordance with established protocols and standards.
• Conduct patient assessments, develop care plans, and collaborate with physicians and the interdisciplinary care team.
• Manage and treat symptoms, including pain and other distressing conditions.
• Support patients and families through education, communication, and compassionate presence.
• Participate in care coordination and case discussions to ensure continuity and quality of care.
• Maintain accurate and timely documentation in compliance with regulatory requirements.

• Master’s degree in nursing (Required).
• 3 years of experience in supervision and independent decision making in major nursing areas (Preferred).
• NP‑C – Adult Nurse Practitioner – AANPCP (Required).
• BCLS – Basic Life Support (Required).
