Primecare Home Care, part of Flournoy Health Systems (FHS), provides compassionate, high‑quality care to clients in their homes, offering skilled nursing, personal care, companionship, and structured family caregiving.

Position Summary

We are looking for a Part‑Time Licensed Practical Nurse (LPN) to provide pediatric home‑care services. The role involves delivering high‑quality nursing care to pediatric patients outside regular hours, supporting families, and collaborating with the clinical team.

Responsibilities
  • Conduct supervisory visits and education in patients’ homes
  • Re‑evaluate patients’ skilled nursing and personal support services needs
  • Work autonomously with minimal assistance from the Clinical Manager
  • Make decisions guided by policies and procedures
  • Prepare clinical progress notes
  • Inform family and Registered Nurse of changes in patients’ conditions or needs
  • Administer medications and treatments as prescribed by a physician, following current medical practice standards
  • Provide total patient care and teach patients/families
  • Perform other services as outlined in the Georgia Licensed Professional Nurse Practice Act
  • Report directly to the Registered Nurse and Clinical Director
  • Maintain patient confidentiality
Qualifications
  • Licensed as a Practical Nurse (LPN) – current, unencumbered nursing license in Georgia
  • Current BLS certification
  • Valid driver’s license
  • Recent TB screening
  • Experience caring for medically fragile individuals
  • Experience in home health (preferred)
  • Experience in pediatrics (preferred)
  • Working knowledge of the Georgia Pediatric Program (GAPP) policies and procedures (preferred)
Compensation

Annual salary: $50,000–$55,000
