Registered Nurse

The Registered Nurse oversees, and coordinates patient care delivered in the home environment. This role blends clinical nursing expertise with care coordination, patient advocacy, and ongoing assessment to ensure patients receive safe, effective, and person-centered care  Registered Nurse works closely with caregivers, physicians, therapists, and community partners to support optimal health outcomes and reduce avoidable hospitalizations.

Responsibilities include:

• Documenting nursing actions of all care given to individuals and informing physicians and family accordingly
• Conducting nursing assessment visits as needed
• Following policies and procedures according to operation, state, and regulatory standards
• Completing supervisory visits based on compliance and regulations to include review of plan of care, caregiver competency assessment, and completion of required documentation
• Implementing nursing support for individuals in a home setting
• Working with Director or Nursing/Nurse Supervisor and/or Branch Manager to address the health needs of individuals
• Participating in staff meetings, in-services, and other meetings as needed
• Ensuring compliance with physician's verbal orders and medical reports

Qualifications include:

• Degree in Nursing from an accredited school
• One or more years of nursing experience preferred
• Current and unrestricted license in the state of the operation
• CPR professional certification
• Excellent communication skills with all levels of personnel, internal and external to the company
• Capable of working independently with highly confidential information

Salary Range: USD $32.00 / Hour
