Job Overview
In the home health care setting, Licensed Practical Nurses (LPN) provide one‑on‑one skilled nursing care and treatment to patients in their home. They assess patient needs and develop a plan of care with the physician, carry out the plan with patient, family, and support staff, monitor and evaluate patient response to treatment, medication, and healing or mobilization progress, and oversee case management.
EssentialFunctions
- Comply with nursing standards under the guidance of the Nursing Staff.
- Maintain clinic, hospital policies, procedures, objectives, quality assurance, safety, environmental and infection control.
- Interpret policies to personnel, patients, and public as appropriate.
- Follow established routine and utilize supplies and equipment in a cost‑effective manner.
- Collect data systematically and continuously; identify issues and develop diagnoses based on assessment data.
- Prioritize data collection based on patient condition and needs; identify expected outcomes and plan based on diagnoses or issues.
- Treat patients and families with compassion; implement the plan of care based on the plan.
- Monitor and document patient response; establish rapport with clients; adapt communication style; listen and respond to patient needs.
- Provide effective interventions and respect patient confidentiality and privacy; support social, spiritual, and cultural needs.
- Manage difficult or emotional patient situations.
- Develop expected outcomes and involve patient/family and other disciplines to determine outcomes.
- Modify expected outcomes based on changes in assessment; involve stakeholders in evaluation; evaluate the plan in relation to patient responses and expected outcomes; document results.
- Document and submit required information and data promptly, using correct terminology and format.
- Adhere to hospital and departmental attendance and punctuality guidelines.
- Perform all job responsibilities in alignment with core values, mission and vision.
- Perform other duties as required and complete all functions per departmental policies and procedures.
- Maintain current knowledge; attend ongoing educational programs.
- Attend staff meetings; complete mandatory in‑services and competency evaluations on time.
- Demonstrate competency in providing care to all patients based on age, sex, weight and demonstrated needs.
- For non‑clinical areas attend training and demonstrate usage of age‑specific customer service skills.
- Wear protective clothing and equipment as appropriate.
- Vocational / Technical Degree:
Graduate of an accredited school of Practical Nursing (Required). - Work Experience:
1‑2 years practical nursing experience (Preferred). - Licenses and
Certifications:
Licensed Practical Nurse (LPN) with current Georgia license (Required);
Basic Life Support (BLS) (Preferred);
Advanced Cardiac Life Support (ACLS) (Preferred). - Must possess and maintain a valid Georgia Driver's License.
