Cancellation Policy:
Affiliate provider personnel may be cancelled up to twelve (12) hours per biweekly pay period. Call offs:
Company has implemented site-specific protocol in accordance with client attendance policies. Temporaries receive a formal complaint after 2 call outs and are terminated after their 4th call out. Temporaries receive a formal complaint after 3 instances of tardiness and are terminated after their 5th instance of tardiness. Termination Policy:
Client may cancel travel assignment without cause, before or during assignment, by giving 14 days' notice to company with no penalty or fee. Client may cancel travel assignment 'For Cause' at any time during travel assignment without penalty or fee to client. For the purpose of this agreement, 'For Cause' is defined as any irrefutable, documented and incurable violation of law or an essential internal policy that has a material negative impact on the hospital or any patient or employee of hospital.
The facility will be solely responsible for determining a 'for Cause' occurrence. Company or any affiliate provider may replace terminated employee without penalty if the replacement is able to start by the next appropriate new employee orientation. Guarantee hours policy:
Client guarantees a minimum of 40 hours per week when 8-hour or 10-hour shifts are worked, or 36 hours per week when 12-hour shifts are worked. These guaranteed hours do not include orientation, on-call or call back hours. Time off and specific scheduling requests honored only if included in initial confirmation of assignment and work order. Client may directly hire any temporary pursuant to the following fee schedule: 30% of year one salary and bonus offered to temporary.
For each consecutive week a temporary is on the initial assignment, the 30% fee will reduce by 2% per week should client choose to bring a temporary on as a client permanent employee prior to ending the first assignment. Furthermore, client may directly hire any temporary without obligation to pay a fee after one 13-week travel assignment.
