• Conduct patient/client/youth intake interviews and create case reports.
• Coordinate and organize delivery of community social services, including referrals to legal, medical, financial, educational, and other support providers.
• Monitor progress toward treatment goals and document outcomes.
• Provide crisis intervention and 24/7/365 youth support, including face‑to‑face response when clinically indicated.
• Develop and implement individualized service plans and life‑skills workshops.
• Act as liaison among the Child and Family Team (CFT) and other agencies, maintaining communication and feedback.
• Prepare intake, case history, and program statistics reports.
• Participate in treatment team planning and team meetings (one face‑to‑face per week).
• Maintain client confidentiality in accordance with agency policy and federal/state law.

• High school diploma or GED and at least two years of experience in a social‑services related position; or a Bachelor's degree in a social‑services related field; or one year at the lower level or equivalent.
• Knowledge of agency policies, confidentiality laws, community resources, state and federal regulations, crisis intervention, and eligibility requirements.
• Strong interviewing, assessment, and case‑management skills.
• Ability to develop and facilitate workshops, service plans, and client profiles.
• Proficiency with agency software and databases for case documentation.
• Clear capacity to coordinate multi‑agency teams and keep detailed records.

• No lifting of more than 20 pounds.
• Must be able to perform tasks with exposure to blood or other infectious materials (Category I) as required by job duties.

Intensive Customized Care Coordination (IC3) is a provider-based, high‑fidelity wraparound intervention that brings together a team selected by the family to identify and achieve goals through collaborative action.
