Position Title
IC3 Care Coordinator
Location & SalaryAlbany, Georgia
Annual salary: up to $30,000 (Grade 11)

Supervisor: IC3 Wraparound Supervisor
- Conduct patient/client/youth intake interviews and create case reports.
- Coordinate and organize delivery of community social services, including referrals to legal, medical, financial, educational, and other support providers.
- Monitor progress toward treatment goals and document outcomes.
- Provide crisis intervention and 24/7/365 youth support, including face‑to‑face response when clinically indicated.
- Develop and implement individualized service plans and life‑skills workshops.
- Act as liaison among the Child and Family Team (CFT) and other agencies, maintaining communication and feedback.
- Prepare intake, case history, and program statistics reports.
- Participate in treatment team planning and team meetings (one face‑to‑face per week).
- Maintain client confidentiality in accordance with agency policy and federal/state law.
- High school diploma or GED and at least two years of experience in a social‑services related position; or a Bachelor's degree in a social‑services related field; or one year at the lower level or equivalent.
- Knowledge of agency policies, confidentiality laws, community resources, state and federal regulations, crisis intervention, and eligibility requirements.
- Strong interviewing, assessment, and case‑management skills.
- Ability to develop and facilitate workshops, service plans, and client profiles.
- Proficiency with agency software and databases for case documentation.
- Clear capacity to coordinate multi‑agency teams and keep detailed records.
- No lifting of more than 20 pounds.
- Must be able to perform tasks with exposure to blood or other infectious materials (Category I) as required by job duties.

Administrative, Direct Care, Medical.
Intensive Customized Care Coordination (IC3) is a provider-based, high‑fidelity wraparound intervention that brings together a team selected by the family to identify and achieve goals through collaborative action.
